It's more of a catchall term these days, up to and including being unmoderated because the admins banned all the moderators. They used to give pretty specific reasons for bans/quarantines/whatever but people kept making fun of them for it and pointing out hypocrisy, so now you just get hyper vague single sentences like "unmoderated" "hate content" etc.
Yes, it's one of reddit's favorite ways to deal with subreddits they don't like. If they don't have a legitimate reason to ban it, they'll just find a reason to ban the mods until they can either slip in one of their own "yes men" to ruin the subreddit or they'll just close the subreddit once it has no more mods.
